The most significant change you will notice is the name. You are now receiving care at East Campus Medical Center at UC San Diego Health.

East Campus Medical Center is UC San Diego Health’s first community hospital, developed to provide high-quality, comprehensive care close to home for people throughout eastern San Diego. You will still receive the same quality care experience you have come to expect from Alvarado Hospital, but now you will also have access to the most advanced treatments, therapies and cures developed by some of the nation’s most prominent physicians and scientists.

Medical Records Requests for Care Received at Alvarado Hospital

Effective Monday, Dec. 11, 2023, UC San Diego Health is the new custodian of medical records at Alvarado Hospital.

UC San Diego Health Information Management will only be responsible for the release of medical records with encounters dated Dec. 11, 2023, and after.

If you require access to your medical records for care received at Alvarado Hospital:

• For a status update on a medical records request submitted to Prime Healthcare (Alvarado Hospital) before Dec. 11, 2023, or to obtain a copy of medical records for care received prior to Dec. 11, 2023, contact Prime Healthcare at 909-657-5767.
• For a status update on a medical records request submitted to East Campus Medical Center at UC San Diego Health on or after Dec. 11, 2023, contact UC San Diego Health Release of Information at 610-994-7500
• If you need to obtain a copy of medical records from East Campus Medical Center at UC San Diego Health for encounters dated Dec. 11, 2023, and after, please download and print a records release form:

  Complete and sign the form, then submit it by one of these methods:
